Dynamic Dependent Drop-downs & Forms

I’m curious as to what’s happening when I have three Dynamic Dependent Drop-Downs, that work perfectly. The problem happens with I wrap them inside a form.

When they are placed inside the form, only the first drop-down displays its contents. The two that follow are blank.

As soon as I remove the , they are display their content.

Do you have suggestions that will lead to a solution?
