Print inside a printed form

Hello! So… In this oportunity I need to create a page to print inside each box from a paper form. I guess I need to use a rule to measure each distance inside the paper form. But… how do I create a printable page with the parameters i give to the page?

I cannot recreate the form digitally because it has a especial paper I cannot print with an ordinary printer
