Help with script on Content Page Include

I have a page that brings if a form via a Content Page Include. The form has CAPTCHA and it only works randomly. It loads without the captcha box about 80% of the time, if I continue to hit refresh it will ultimately show a couple of times then goes away again. I have the captcha script in the head of the template page. Are there any tricks to using scripts on these. Also, have a form with phone validation via javascript. It works if the form is on the template page, but not if on the Content Page Include.

I’m new to Page Content Includes and am just discovering the power of them, awesome!! Here are images showing both loaded and uploaded.
